If the marketing blurb says „active holidays“ while the exclusions say mountaineering, the insurer is under no obligation to guess that you meant Elbrus.

Mountain Guide has worked on Elbrus since 2001 and runs programmes worldwide. Policy requirements differ: you cannot use one checklist for walks around the Elbrus region, Elbrus from the south, the self-sufficient western route and an 8000-metre peak, simply changing the name of the country.

Checking a policy in two minutes

The policy is valid in every country on the route and in transit.

The dates cover the whole period from departure to return, not only the days on the mountain.

The activity named expressly includes trekking, mountaineering, ski touring or whatever else the programme involves.

The maximum permitted altitude is no lower than the highest point of the route.

Medical care and hospital treatment are covered.

Search and rescue is covered, if the route lies beyond quick evacuation by road.

Ground evacuation is covered, and helicopter evacuation where that is relevant.

The limit for rescue work and for the helicopter is not buried inside the overall sum at a much lower level.

Medical repatriation and repatriation of remains are covered.

Chronic conditions and any other circumstances the insurer requires you to disclose have been declared.

There is a round-the-clock assistance line and a clear procedure for approving costs.

If even one of these points cannot be confirmed by the text of the policy or by a written answer from the insurer, treat it as unconfirmed. A spoken „yes, it all covers you“ sounds fine in the office and markedly worse to an evacuation controller.

What the activity has to be called

The name of the programme on the website and the insurer’s own classification may differ. The document must expressly permit mountaineering or the corresponding mountain ascent, search and rescue, and evacuation. On the self-sufficient routes from the north and the west, the arrangements for organising rescue matter especially, because transport access and infrastructure differ from the classic south.

The word „helicopter“ appearing in a policy does not mean a helicopter will be able to fly in any weather and to any altitude. Insurance pays for the service provided for; whether the flight is physically possible is decided by the weather, the landing site, the availability of an aircraft and the rescue service.

The overall sum insured and the individual limits

A policy can look impressive on the headline sum while capping the helicopter, search work, dentistry, outpatient care or a flare-up of a chronic condition with separate sub-limits. It is the table of limits and exclusions that determines the real cover.

the overall medical sum;

search and rescue;

the helicopter and other evacuation;

in-patient treatment;

transport to a suitable medical facility;

the journey home after treatment;

repatriation of remains;

the excess, and the costs you have to pay yourself first.

Why the assistance service matters

Insurers are often chosen on the price of the policy, but in an incident what you deal with first is the service company: the assistance provider. It takes the call, checks the cover, and arranges the clinic, the transport and the guarantee of payment. You need to know in advance:

whether the line operates round the clock;

whether you can reach it by phone and over the internet;

whether approval is needed before a helicopter is called;

who issues the guarantee of payment;

what to do if there is no signal or help is needed immediately;

which documents and medical reports will be required.

The exclusions people miss most often

sport or mountaineering is not included in the basic tariff;

the route goes above the permitted altitude;

off-piste skiing or ski touring is excluded;

search work is not covered if the person’s whereabouts are unknown;

rescue is paid for only where there is a threat to life on medical grounds;

there are restrictions for competitions, unguided routes or the absence of a licensed guide;

chronic conditions have not been declared;

the claim involves alcohol or a breach of the law;

the policy was bought after departure and carries a waiting period.

What to keep before the trip

the full text of the policy, not just the smart-looking card;

the table of cover and exclusions;

the insurer’s written confirmation of the route and the altitude;

the policy number and the assistance contacts;

copies of the documents on your phone and in the cloud;

a next-of-kin contact;

a list of diagnoses, allergies and medicines you take regularly.

The policy and phone numbers must be available offline. An email link that only works with a good internet connection is of little use in the mountains.

What to do if something happens

Tell your guide and do whatever immediate safety requires.

Contact the assistance service at the first safe opportunity.

Give the policy number, your location, the condition of the casualty and the evacuation options available.

Get a case number and write down names, times and decisions.

Confirm the clinic and costs with the assistance service if the situation allows you to wait for approval.

Keep receipts, medical documents, transport invoices and correspondence.

Where life is in immediate danger, the first task is help and evacuation, not perfect paperwork. But as soon as things are stable the assistance service has to be brought in: approval obtained late can complicate reimbursement.

What this checklist rests on

For organised treks and expeditions the UIAA Medical Commission recommends comprehensive cover including search and rescue, a helicopter where necessary, medical care and repatriation, taking account of the particular country, the altitude and the risks. Official advice for travellers also stresses that ordinary medical insurance and evacuation cover are two different things.

UIAA: Quality of Organized Treks or Expeditions

Travel.State.gov: Travel Insurance

GOV.UK: Foreign Travel Insurance

This page helps you work out what to ask an insurer; it does not interpret any particular contract. But then there is a clot, or an inflamed gall bladder or appendix, or a stray rock, or an unlucky trip on the trail, pulmonary oedema, cerebral oedema, or simply a bad chill caught at altitude.

And then comes the moment to get acquainted with the medical system of the country you are in. Better to do that at the insurer’s expense than out of your own hard-earned money, quite apart from the fact that going through the insurer is many times faster, and time is often measured in hours. And, of course, no serious company, Mountain Guide included, will take you anywhere without proper insurance; we do not need adventures of that kind.

DO NOT ECONOMISE ON INSURANCE! For climbs on serious peaks I would not set a limit below 100,000, and for treks below 50,000. In 2024, for example, the rates in Nepal were as follows:

medical helicopter evacuation in Pakistan from 5000 m – from 27000 dollars

medical helicopter evacuation in Nepal from 6800 m – 23000 dollars

medical helicopter evacuation in Nepal from 4000 m, the Kanchenjunga area – 9500 dollars

medical helicopter evacuation in Nepal from 5000 m, the Everest area – more than 5000 dollars

a day in hospital – 2800 dollars, without serious procedures or operations.

a single search flight around a mountain by helicopter, a few minutes – 10000 dollars

evacuation of the bodies of dead climbers from 7000 m, not counting the search work, plus repatriation – from 36000.

The limit on helicopter payments deserves particular attention. Often a policy for 50,000 includes the helicopter for only 5000. In the Everest area that will just about do; in other regions it will not. And although you can often get out from below Annapurna by jeep, on very bad roads, acute appendicitis gives you only a few hours to reach a hospital.

On climbs of serious peaks it is also worth checking carefully whether search and rescue is in your policy at all: often it is not.

Nepal

Instructions for arranging insurance through the online service www.saveprolife.ru for trekking or mountaineering in Nepal.
